Officer Service Record (NAVPERS 1070/66)

Purpose

The service record is a chronological record of events of an individual during his or her naval service. The service record is the official history of a member's service in the Navy and is the property of the Government and not the member. Service record pages are a vital and permanent part of the record, and entries may not be made except as authorized by the commanding officer.

    Navy Terminology

    Explanation

    NAVPERS 1070/613 — Administrative Remarks (Rev.10-81) Copy of any messages in reference to Permanent Change of Station (PCS) orders, availability, TDY, LIMDU, hospitalization, deaths, illness, etc.
    NAVPERS 1070/605 — History of Assignments (Rev. 10-89) (current) List of duty stations, Planned Rotation Date (PRD) adjustments, change of rate.
    NAVPERS 1301/51 — Officer Data Card Current status, list such items as college graduated from and type of degree, Navy schooling attended, warfare specialist, etc.
    NAVPERS 1210/5 — Officer Qualifications Questionnaire This space left intentionally blank
    SGLI 8286 — Servicemen's Group Life Insurance Election and Certification (Rev. 11-92) Life Insurance coverage amount selected, names of beneficiaries, and percentage of insurance benefits each is to receive. Declination notice if you do not want any coverage.
    DD 214 — Certificate of Release or Discharge from Active Duty (all) Shows prior enlisted service information.

OPNAV 5211/19 — Record of Disclosure, Privacy Act of 1974 (Rev. 8-81) Signature of person reviewing/implementing information into service record and date of entry.
OPNAV 5520/20 — Certificate of Personnel Security Investigation, Clearance and Access (Rev. 10-79) Results of #19 through #22, with assigned level of clearance.

The Officer Service Record is a valuable collection of information vital to your naval career. Ensuring that the proper personnel maintain and safeguard it should be of concern to each of us. In most commands, the service records are located at and properly maintained by the supporting personnel support detachment or activity (PSD/PSA) or the personnel office in the case of operational or afloat commands.
